Based on your results, we recommend contacting a care provider to see if post-COVID rehabilitation could help with symptom management. The provider can then refer you to Madonna’s Post-COVID Assessment and Rehabilitation Clinic.
These results are meant to inform a conversation with your physician or medical care provider. The results of the assessment do not constitute a medical diagnosis.
If you think you are a candidate for post-COVID rehabilitation, consult a healthcare provider for a referral. Going over the results of your self-assessment, your history with COVID and your symptoms sent to your email is the first step.

Madonna Rehabilitation Hospitals offers a setting for individuals who may be affected by post-COVID to find more information, ask questions and receive support.
Many find that sharing their story both brings about their own sense of relief, but also empowers others in the group. This feeling of community can be immeasurably useful in the healing process. Post-COVID support groups are offered on both our Lincoln and Omaha campuses, as well as Zoom options. Groups are open to the public and participants are not required to be a patient at Madonna.
